Licensed HVAC Contractor Application Form
Energy Services is extending funding to Michigan based licensed HVAC contractors to complete an onsite HVAC inspection checklist to make recommendations to reduce infectious aerosol transmission at K-12 public school buildings. The recommendations must include cost estimates for implementation. Both the school building to be inspected and the HVAC contractor completing the inspection must be pre-approved by EGLE Energy Services prior to delivering assistance under this program. This is to ensure program integrity, and to help as many school districts as possible.
Instructions: Complete the form below and attach required documentation. To be considered for funding, an application must be submitted and an award issued prior to assisting a school district.
Funding Notification and Reimbursement Request: Applicants will be notified of the funding decision by email. Funds will be dispersed only on a reimbursement basis. Only expenses incurred after notification of award are eligible for reimbursement and the payment request must be submitted no later than 10 days following completion of the HVAC inspection checklist report.
