Introduction
Purpose
Georgia Family Connection Partnership developed the Collaborative Vitality Survey as a systematic way for individuals to express their experience as Collaborative members. Collaboratives can use survey findings to help acknowledge and celebrate achievements while identifying areas to improve and grow.

Completing the Survey
The 10-minute survey is a voluntary, anonymous, personal way to assess perceptions of Collaborative performance from a broad cross-section of members. GaFCP does not use the findings to sanction or reward your Collaborative, so please be candid.
It’s up to you to decide how to interpret each question. If you’re not sure what a question means, take your best guess or check “don’t know.” If you’re a member of multiple Family Connection Collaboratives, consider just the Collaborative that invited you to complete the survey. You may complete one survey for each Collaborative that invites you to participate.

What's Next?
GaFCP staff will compile and analyze all completed surveys. We’ll provide you with customized findings about your Collaborative members’ responses in late 2024. We will not share your Collaborative's findings with anyone else.
