Survey Introduction and Informed Consent

You are invited to participate in a research study looking at substance use, its effects, and community views on substance use in the Catskills region. The results from this study will help to inform and direct resources and programs focusing on reducing the impact of substance use across our communities.
This study is being conducted by: Dr. Simon J. Purdy, Associate Professor of Criminal Justice at SUNY Delhi, on behalf of the Catskills RCORP Consortium, a group of local agencies and organizations whose goal is to help reduce the harms associated with the use of opioids and other substances in our local communities.
If you agree to take part in this study, and you are at least 18 years old, you will be asked to answer a set of survey questions in this online survey form about your experiences with substance use, its impact on your life and the lives of those around you, your views on substances in your community, and several demographic questions. This survey should take no more than 30 minutes of your time. There are no major risks involved with your participation in this study. No personal or identifying information will be collected (not even your email or IP addresses), meaning that your responses, are anonymous and cannot be linked back to you in any way. Participation in this study is completely voluntary. If you decide to participate, you are free to not answer any questions you do not wish to answer, or withdraw at any time.
While there are no direct personal benefits of participating in this study (i.e. you’re not getting paid, etc.), the data collected from you will be used to create and expand programming in communities such as yours. In this way, your participation in the study is helping to build stronger communities and assist those in need throughout the Catskills Region.
