Registration for Yawarani, Brazil Trip - August 31st - September 8th, 2025

4.Have you previously attended ceremony with Hummingbird Church? - If you have not you will be sent an Intake Form to complete prior to approval to attend this trip. If you have already attended ceremony with Hummingbird Church you will not be required to complete an Intake Form, you will be sent a deposit invoice once we receive this form.(Required.)
5.I understand the following:

- The total cost of the trip is $2,000
- My deposit is $1,000 and must be paid by April 1st, 2025, this is non-refundable and non-transferable.
- My final payment of $1,000 is due by August 1st, 2025
- All funds collected will be sent directly to the Yawanawa of the Yawanari village. Hummingbird Church does not even have the option of refunding me this money.
- I have a passport or will have a valid passport prior to this trip
- I must bring my own sleeping supplies
- I must book my own flight into and out of CZS arriving at 11:55 AM on August 31st, 2025 and departing CZS on September 8th, 2025.
- Airfare is not included in the cost of this trip.(Required.)
