Sweet Home Schools Parent Survey |
About this survey:
About this survey
The Department of Urban and Regional Planning at the University at Buffalo is conducting a research study on the relationship between physical, social, neighborhood, and cultural environments and children’s commuting habits and well-being.
You are receiving a request to complete this survey because you are a primary caregiver for a child studying in grades K-8. We ask that you (the caregiver) complete only one survey per child. If you have more than one child who attends these grades, you will receive a separate request to complete a survey for each child.
Your answers to this survey are confidential and will not be shared with anyone except the research investigators and research staff. There is likely no direct benefit to you for participating in this study, but it will help researchers (including the UB Food Systems Planning Lab), city officials and planners (such as the Town of Amherst), and school districts (such as the Sweet Home Central School District) to create places that support children’s well-being. Survey results will not be used for any purpose besides research, and only aggregated results will be reported in research results.
For Further Information
If you have any questions, concerns or complaints about this study, please call 716-829-6289 or email foodsystems@ap.buffalo.edu.

This survey will take about 15 to 20 minutes to complete. Thank you for your time.
PARTICIPATION IN THIS SURVEY IS VOLUNTARY. YOUR CONSENT TO PARTICIPATE IS IMPLIED BY YOU ANSWERING ANY OF THE QUESTIONS IN SURVEY.
