CLARENCE SOCCER TRAVEL REFUND FORM

The purpose of this survey is for families to let us know how they would like to be refunded for the 2019-20 travel season. While the CSC endured several expense and overhead November 2019 – March 2020, we have decided to do right by our families and offer the following:

All families will receive the $75 volunteer refund (per family), which will be credited on the card used to register for the travel season.

Families will also have the option to make a tax deductible donation for the volunteer fee to the CSC. We will email you a letter confirming the charitable contribution amount.

All families have the option to select one of the following in relation to the travel season being shortened:

$100 refund per player (this is in addition to the $75 volunteer fee) which will be credited on the card used to register for the travel season.

-OR-

$125 credit per player towards the 2020-21 travel season (this is in addition to the $75 volunteer fee). If you select this option, you will be emailed a unique discount code that can be applied at checkout when registering for travel next season.

Families have the option to make a tax deductible donation for $100 per player to the CSC. We will email you a letter confirming the charitable contribution amount.
