Thank you for your voluntary participation in this survey to help assess the community needs for people with disabilities. The results of this survey will help the Southwest Center for Independent Living (SCIL) evaluate current programs and services, as well as, assist in planning and prioritizing for our future programs and services in our service area in these southwest Missouri counties: Christian, Dallas, Greene, Lawrence, Polk, Stone, Taney and Webster.
The information collected is anonymous and your identity and participation will remain confidential. You are under no obligation to take this survey, but your input is appreciated.
If you share your contact information, you will be entered to win a $50 gift card. The winner will be announced on 10/05/16 at 3pm.
A printed copy can be obtained at our offices or download at www.swcil.org. Please mail or drop off at our main office.
The survey will take approximately five minutes to complete. Questions are in four sections: Community Needs for People with Disabilities, SCIL Awareness, Charity Support, Demographics.
