Thank you for submitting a nomination for the second annual CNBC Changemakers List, celebrating women transforming business.

Please complete this nomination survey by 11:59 PM ET November 11, 2024.
The final list will be selected using a data-driven approach that analyzes the impact and significance of each nominee's contribution to their industry or sector in calendar year 2024.
To be eligible for inclusion on the 2025 CNBC Changemakers List, women must be in a leadership role at a company or organization that meets the following recommended minimum qualifications:
- For private companies and organizations (including non-profits): at least $25 million in revenue in at least one of the past 3 years OR an enterprise value of at least $100 million.
- For public companies: market capitalization of at least $250 million
For purposes of the CNBC Changemakers List, a "leadership role" is defined as:
- Founder
- CEO
- President
- Other C-suite role (CFO, COO, CIO, CTO, CISO, CHRO, etc. or equivalent title, reporting to the CEO and/or the Board of Directors)
- Chair or member of the board of directors of an organization that meets the above eligibility requirements
Anyone with access to the information needed to complete the form accurately may nominate someone for the list. This can be a PR or communications representative of the nominee or her company, a colleague, mentor or protégé of the nominee, or the nominee herself.
Note: these are recommended minimums, but we will make exceptions for those in non-traditional businesses or with especially compelling transformational stories.
