OPEN ACCESS MODEL ASSESSMENT

The foundation of the Open Access Model is based on behavioral health organizations completing the intake and assessment on the same or next business day once a referral has been received.

The CMAG team of consultants designed this assessment tool to assist providers in determining how prepared they are to implement the Open Access Model. This assessment is based on our work with over 30 behavioral health organizations to implement this model.

Once you have completed the assessment, you will receive a score. The higher the score, the more prepared your organization is to implement the Open Access model. Do not worry if the score is not as high as you would like. The CMAG team has worked with many behavioral health organizations to successfully implement the model that originally scored low on the assessment.

Once you have completed this assessment, a member of the CMAG team will be in contact to discuss your score and answer any questions you may have.
