Hello! Thanks for helping with this survey to support Rhode Island's Olmstead Plan. An Olmstead plan is a way for states to help people with disabilities have the opportunity to live in the community, like their friends and neighbors, as the law says they should. Your answers to this survey will help us understand the needs of people with disabilities in Rhode Island. It's going to take about 20 minutes to finish.
Before you start, here are some important things to remember:
This is a chance for you to make sure your voice is heard. The Executive Office of Health and Human Services (EOHHS) uses information from this survey and other surveys to better understand your needs. We also use information from surveys done by our partners to help us meet the needs of people with disabilities.
We ask for your contact information at the end of the survey. You don’t have to give it to us if you don’t want to. If you do provide your contact information, we won’t connect it to your other answers when sharing survey results.
