Communities for Healthy Aging Transitions (CHAT) is a regional project to look at age-friendliness in Strafford County, as well as Brookfield, Wakefield, Newmarket, Nottingham and Northwood in Carroll and Rockingham counties, respectively. This survey allows us to CHAT with those who know their communities best, the residents. CHAT is a partnership between Strafford Regional Planning Commission and Greater Seacoast Community Health. We both look forward to hearing from you on what makes your community age friendly, which we define as communities being inclusive, welcoming, supportive, and healthy environments for people of all ages.
