Community Hospital Fairfax - 2025 Community Health Needs Assessment Stakeholder Feedback

Community Hospital Fairfax along with area community leaders have begun the process of updating a comprehensive community-wide 2025 Community Health Needs Assessment (CHNA) to identify unmet health needs. To gather current area feedback, a short online survey has been created to evaluate current community health needs and delivery. NOTE: Please consider your answers to the survey questions as it relates to ALL healthcare services in our community, including but not limited to our local hospital.
