High Quality Ideas and Promising Practices for Dementia in Canada
THANK YOU for participating in this important survey designed to identify high quality dementia programs and practices in Canada.
Survey answers will be shared with the Canadian Foundation for Healthcare Improvement (CFHI), a not-for-profit organization funded by Health Canada that supports the rapid adoption of proven healthcare innovations.
The intention is to create a Canadian quality improvement collaborative to accelerate the spread of programs and practices that optimize the experience of people living with dementia and their care partners.
Updates on the initiative will be available on the cfhi website over the next few months: www.cfhi-fcass.ca
All your answers are confidential and your contact information will not be used without your consent. This survey will take about 10 minutes to complete.
