The PA-509 Continuum of Care's Connect To Home: Coordinated Entry System of Eastern PA​ is seeking qualified organizations to serve as Coordinated Entry Access Sites.

The Connect To Home: Coordinated Entry System Of Eastern PA (CES) coordinates and manages access, assessment, prioritization and referral to housing and services for any person(s) experiencing or at imminent risk of homelessness in the following counties in Pennsylvania: Adams, Bedford, Blair, Bradford, Cambria, Carbon, Centre, Clinton, Columbia, Cumberland, Franklin, Fulton, Huntingdon, Juniata, Lebanon, Lehigh, Lycoming, Mifflin, Monroe, Montour, Northampton, Northumberland, Perry, Pike, Schuylkill, Somerset, Snyder, Sullivan, Susquehanna, Tioga, Union, Wayne And Wyoming.

Coordinated Entry Access Sites are the physical “front doors” into the Eastern Pennsylvania Continuum of Care housing crisis response system.

Please note that the CoC will provide training and licensing for Coordinated Entry Specialists but does not have additional funding for new Access Sites at this time. Applicants will need to secure their own funding for Access Site operations or provide them in-kind. However, if your organization already provides some sort of intake and referral to people experiencing or at risk for homelessness, there is benefit for both your staff and clients to being part of a coordinated regional network that connects people to housing, emergency shelter and homeless prevention programs.

While housing and homeless service providers would make excellent Access Sites, other organizations that serve people experiencing or at risk for homelessness are welcome and encouraged to apply, including:
  • First responders
  • Critical Time Intervention (CTI) and other mobile crisis providers
  • Homeless Street Outreach providers
  • Health and human service providers
  • County information and referral agencies
  • Re-entry providers
  • Transition Age Youth service providers
Priority will be given to applicants based in communities with a higher concentration of need and those in which Access Sites do not already exist.

According to the National Alliance to End Homelessness, Housing First is a homeless assistance approach that prioritizes providing permanent housing to people experiencing homelessness, thus ending their homelessness and serving as a platform from which they can pursue personal goals and improve their quality of life. This approach is guided by the belief that people need basic necessities like food and a place to live before attending to anything less critical, such as getting a job, budgeting properly, or attending to substance use issues. Additionally, Housing First is based on the theory that client choice is valuable in housing selection and supportive service participation, and that exercising that choice is likely to make a client more successful in remaining housed and improving their life.
