Case 1
An 86 year old patient was admitted with COPD and who also has a history of chronic kidney disease and a-fib. The patient had a Foley placed at an outside facility. What is the first step you would do when receiving a patient with a Foley from an outside facility?
Place an order for the Foley in the system for POA catheter
Clarify with the patient if they had it at home prior to admission
Ask the physician if it is to be maintained or if it can be removed
Nothing the patient has CKD, so we should maintain it

Case 2
A patient with a PMH of COPD, PNA, BPH, TIA and a-fib was admitted to your unit for shortness of breath. The patient has consults for Urology, Pulmonary with the attending being the Hospitalists. The patient had a Foley placed by Urology on the day of admission for retention. The order has the Nurse Driven Protocol ordered by the Hospitalists. The patient has now had the Foley for 8 days and getting ready for discharge. What would you do about the catheter?
Look at the order and see who ordered the catheter then ask them what to do
Nothing, chart that it is to be maintained due to the urology consult
Review the last Urology note to see what they said about the catheter
Call Urology and ask if they can remove the catheter
Just take it out because you don’t think the patient needs it

Case 2 Continued
You find that the patient’s last Urology notes state that they are signing off and the Foley is to be maintained per the attending team. The patient has been charted as having the catheter being maintained for Urology by the previous shift RN. What steps would you take?
Call the attending to get a removal order to take out the Foley
Maintain the Foley per the Urology note
Call Urology to clarify the order
Discontinue it per the protocol if appropriate
