Lourdes Rural Health Access Survey

          Thank you for taking the time to complete this survey, which is part of Lourdes Hospital’s response to their recent Community Health Needs Assessment. We welcome your responses to this survey, even if you are not a Lourdes patient, for the goal is to learn about the health-related needs of rural community residents.

          The information that you provide will be used to identify ways that Lourdes Hospital can better serve your community.

          Your responses to this survey will be kept private and your confidentiality will be protected. Your responses will not be connected to any identifying information. This survey will be used by Lourdes  to help improve services in rural health.
