Please Note: Questions relating to services provided include all severities of brain injury - mild, moderate and severe, with brain injury defined as: the multiple disabilities arising from damage to the brain acquired AFTER birth. It results in deterioration in cognitive, physical, emotional or independent functioning, and can be as a result of accidents (trauma), stroke, brain tumours, infection, poisoning, lack of oxygen, progressive neurological disease etc.
