---Candidate Agreement---

I understand that membership is contingent upon successful completion of the candidate process and reaching membership eligibility status. If I am not accepted on the first attempt, I may re-attempt a second time (see Articles IV and V of the Delta Psi Constitution).

Beta Alpha Psi has my permission to review my grades to determine my eligibility for candidate and membership status. In addition to academic eligibility, active participation in Delta Psi Chapter activities during my candidacy is a prerequisite to initiation into the Delta Psi Chapter at Cleveland State University (est. 1977).

After initiation, I agree to play an active role in the Chapter.
If I decide to drop the membership, I will do so in writing and return the certification to the Delta Psi Chapter.
I further understand that all fees and dues are non-refundable.
