About this survey
Age Scotland and About Dementia’s policy team are looking at issues surrounding the uptake and use of power of attorney documents in Scotland. They are keen to try and establish a picture of the current uptake of and to hear thoughts on some ideas they have to improve the use of it.

This survey is for people living with dementia or the family/friends/carers of someone living with dementia ONLY

The survey should take under 10 minutes to complete.
