Transitioning into adult health care

Transitioning into adult health care is an important part of your child's medical care. By answering the questions below, you will help us build programs and services around successful health care transition for the children in our community.

This survey is only for parents of children currently age 14 - 21, who have already had epilepsy surgery, and are still under the care of pediatric health care providers.

9.Have any of your child's health care providers explained to you the changes that happen in health care starting at age 18 (such as changes in privacy, consent, access to health records, or making decisions?)
10.Have any of your child's current doctors helped you or your child find a new adult doctor or other health care provider to move to?
11.Have any of your child's current health care providers helped your child gain skills to manage their own health and health care (e.g. understanding current health needs, recognizing signs of seizures or hydrocephalus, knowing what to do in a medical emergency, taking medicines)?
