Welcome to the Break Free Together Dinner Experience Registration
Hello,
We are excited that you are interested in attending the Break Free Together Dinner Experience! The first part of the following questionnaire is to register you for the event and to get information so that we can make this as positive an experience for you as possible. The second part is a pre-survey that will help us measure the impact of this program (you will get a post-survey a few weeks after the event).
We have collaborated with a researcher who is going to use the data from the pre- and post-surveys to help us understand the program effectiveness. However, we will only ever see aggregate data and will not be able to link anyone’s name to any information. The researcher will keep all information completely confidential and secure. Taking part in the research portion of this questionnaire is completely voluntary (we will indicate where that part starts). We sincerely hope you will take the time to fill out the entire survey, but you can choose not to take part in the pre-survey.
The pre-survey will take approximately 20 minutes to complete. There are no known risks to filling out this survey; however, you may stop taking it at any point, if you so desire, and there is no penalty for doing so. There are also no known benefits for filling out the survey, but some people report enjoying being able to share their thoughts confidentially.
This research has been approved by the Institutional Review Board at Maryville College.
