2017 Post-Workshop Survey

* 1. Greetings from the 2017 Broadening Participation Planning Committee! We are delighted that you have decided to participate in our workshop this year! It is important to us that we learn a little more about each of our participants so we invite you to complete the survey that follows.

The survey consists of several questions that asks about your current occupation, activities, and your perspective on a few items. The survey should take approximately 10 minutes to complete.

Your participation in this survey is voluntary and you may refuse to participate or stop the survey at any time. Because this is an on-line survey, we are not able to get written documentation of your consent.
