We are seeking feedback from individuals and families supported by The Arc.  Please answer the questions contained in the survey based on your experiences with The Arc’s supports.  Please answer each question as accurately as possible.  Your answers will be kept confidential and comments are encourages and may be added where indicated.  Results will be reviewed and shared within The Arc community so that we can continue to strive to provide life enriching experiences for the individuals we support.
