Background
BAPAM is a unique medical charity providing health advice and support to performing arts professionals and students through free assessments in our clinics and a Directory of practitioners with a special interest in performer health.
We continuously monitor all our services. We would be grateful if you could complete this anonymous survey to tell us about your experience with a healthcare professional registered on the BAPAM Directory of Practitioners.
BAPAM Directory practitioners work independently from BAPAM but have a special interest in performer healthcare, and are involved in BAPAM training and other development initiatives in performer healthcare in the UK. The information you provide will help us develop this important area of work.
The maximum number of questions you will be asked is 19.
You will need to complete a new form for each practitioner seen.
The survey is available online at www.surveymonkey.com/s/bapamdirectory. You can also return a hard copy to BAPAM, 7 - 9 Breams Buildings, London, EC4A 1DT.
