NHS Bristol, North Somerset and South Gloucestershire Integrated Care Board is a group of people who are in charge of running the NHS in these areas. They are responsible for thinking about what people need, making sure they have the right services and looking after the finances.
NDTi want to find out what is important to people who are autistic or have a learning disability, living in Bristol, North Somerset and South Gloucestershire.
NDTi have been asked by the Integrated Care Board to find out what is important to people who are autistic or have a learning disability, living in Bristol, North Somerset and South Gloucestershire.

We will use this information to write a learning disability and autism needs assessment. This is to help commissioners in health and social care to make decisions.

We want to hear what is important to:
• People who have a learning disability
• Autistic people
• Family carers
• Paid supporters

To help everyone to have their say we are:
• doing a survey
• having conversations with people at different places in the area
• working with local people who have a learning disability or are autistic as part of our team.

After asking people questions, we will tell the Integrated Care Board and other interested people what we find out.
We hope that what we find out will help services to plan what people want and need in the future.

This survey has between 12 and 17 questions, depending on your answers.

You can decide if you want to take part or not.

The survey is confidential.
Only people within the research team will know who has taken part.

We might use some quotes from what you said in our report, but we will not use your name.

At the end of the survey, we will ask you if you want to be entered into a prize draw with the chance to win a voucher worth £25.

If you want to enter the prize draw, we will ask you to give an email address or phone number we can contact you on.
We will randomly select five winners. Not everyone will win a voucher.

If you change your mind about taking part, you can ask us to remove your answers. You can do this until the end of June.
After this time, we might not be able to remove your answers if we have written a report.

We can only remove your answers if you have provided an email address or phone number. Otherwise we will not be able to tell who said what.
