“You Are Having An Angiogram…You Have Treatment Choices…”

If we gave you this information before your angiogram, would it have been useful?

The patient education pamphlet you have kindly reviewed entitled “You Are Having An Angiogram…You Have Treatment Choices…” was made to help patients who are coming in from home to have an elective angiogram to understand the following:

·       What the doctor is looking for during an Angiogram

·        Coronary Artery Disease and Stable Angina

·       How to treat Stable Angina caused by Coronary Artery Disease

·       Things the doctor thinks about when suggesting the types of treatment plans available for each patient

·       The patient’s right to learn all possible treatment options including the benefits and risks of each, such as survival, relief of Angina, quality of life, and risk of re-intervention...).

·       The patient’s right to be included in choosing the treatment plan

·       The patient’s right to include family or friends in picking the treatment plan.

·       We are asking for patients opinions to see how well the Pamphlet teaches patients the information and tell us how it can be improved.

Please circle the heart number that you feel best shows how each section/paragraph is explained: The first heart would be the least well communicated and the fifth heart would be the best.
Thank you
