The Client-Trainer agreement is set out on Page 2 and lists the terms and conditions of training with me, Angele Satariano. By clicking 'Submit' at the end of this online form, you confirm your agreement to such terms and conditions.

If you are between the age of 15 and 69, the Physical Activity Readiness Questionnaire is important to ensure that it is safe for you to exercise.

If you are over the age of 69 and don't lead an active lifestyle, please check with your doctor before changing your levels of physical activity.
