Easy Read & Plain Language Resource Survey

Instructions: This is a survey to help the Autistic Self Advocacy Network (ASAN) learn about how people use our Easy Read and plain language toolkits. A survey is a tool that asks questions about a topic to get information. We want you to take this survey to help us understand more about how you used our toolkits. 

There are 8 questions on this survey. Some of these questions are required. “Required” means you have to answer the questions. We marked required questions with the word “required” and a red star (*). Other questions are optional. “Optional” means you only need to answer the questions if you want to. 

Please take the survey once for each toolkit you use. If you used multiple toolkits, take the survey again for each toolkit. You can take the survey again by visiting surveymonkey.com/r/ASANresource.
