Alternative Dispute Resolution Program - Participant Survey

Purpose
To monitor and improve the quality of the ADR program, the ADR Advisory Committee of the U.S. District Court for the Eastern District of Missouri developed a survey for participants to complete at the conclusion of ADR. The survey is brief and should not take more than ten minutes to complete. The survey provides participants in ADR the opportunity to let the Court know what they thought of their experience in ADR.

Confidentiality
The completed surveys are only accessible to the Court’s ADR staff. The responses of ADR participants will be used to generate information concerning the effectiveness of the ADR program. All information provided in the survey will remain strictly confidential.

Instructions
The Court encourages each participant in a case that went to mediation to complete the survey. Neutrals should not complete a survey. A participant is defined as a plaintiff, defendant, attorney of record, insurance representative, in-house counsel, a representative from the department of human relations, corporate representative, or a GAL/next friend. Each participant is encouraged to complete and submit a survey. Limit one survey per participant. Please select or write-in only one answer per question. At the bottom of each page, please select "Next" to advance to the next page. When you have completed the survey, please select "Done" at the bottom of the last page and that will submit your survey.
