The Department of Human Services, Division of Substance Use, Prevention and Recovery is requesting disclosure of information that is necessary to accomplish the statutory purposes as provided in Ill. INTRODUCTION
Funds awarded by the Illinois Department of Human Services Division of Substance Use, Prevention and Recovery (IDHS/SUPR) will support deliverables as identified in the contractual agreement(s).
Organizations that enter into a contract with IDHS/SUPR must complete an Annual Certification Plan (ACP). The FY 2021 ACP, when certified by the organization and accepted by IDHS/SUPR, formalizes service and funding expectations contracted between IDHS/SUPR and the organization.
A complete electronic submission with all related forms and document is due no later than end of business on February 15, 2021. Applications will not be considered complete unless all questions are answered and applicable forms are updated and emailed. For questions or concerns contact your contract manager.
