INSTRUCTIONS
There are four sections in this survey:
Section A - Background Information
Section B - Job Tasks
Section C - Knowledge and Abilities
Section D - Core Compentencies
In section A you will be asked to provide information about yourself and your experience in wastewater collection. As part of this section, you will be asked to provide information about the plant at which you are currently working.
In sections B and C you will be asked to provide ratings on the SERIOUSNESS and FREQUENCY scales listed below. These ratings should be based on your experience as an operator. If you are not currently working as an operator, please respond based on the typical operator.
SERIOUSNESS
The seriousness of consequences that arise from inadequate or incorrect performance:
- Task not performed
- Negligible or trivial
- Not too serious
- Somewhat serious
- Average seriousness
- Above average seriousness
- Extremely serious
- Disastrous (e.g., life threatening)
FREQUENCY
I perform this task:
- Task not performed
- About once every year
- About once every six months
- About once each month
- About once each week
- About every other day
- About once each day
- About once each hour or more often
In section D you will be asked to rate the relative importance of several core competencies.
If you do not have time to complete the survey in one sitting, you can stop and complete the survey later (provided you use the same computer and have cookies enabled on that computer). The survey will resume where you stopped.
