AAPath 2024 CME Evaluation Form

Instructions and Individual Information

IMPORTANT: Due to the restrictions of our online system, users are unable to save their progress and return to the form later. As such, you should complete the full CME Learning Tracker beforehand, then input your answers online in a single sitting. All submissions must be received by September 30, 2024.

You MUST complete all areas of this CME Evaluation to the best of your ability. Partially completed forms or forms with credit hours without an evaluation of the educational objective, will not be accepted for credit. Please note that by selecting 0.00 credit hours, you are indicating that you did not attend the session or view the recorded content. Physicians should claim only the credit commensurate with the extent of their participation in the activity. By submitting your form, you attest that you have viewed the presentations for which you have selected and evaluated.

Submit responses online no later than September 30, 2024. All submissions are reviewed by staff prior to issuing CME certificates. Certificates will be sent to the email address provided on the form within 4-6 weeks of submitting your claims. Certificates will be issued on a monthly basis through the week following the deadline of September 30, 2024.
