Introduction

Welcome to the HFSA Heart Failure Provider Job Analysis Survey

The purpose of this survey is to identify the significance of tasks performed by the Heart Failure Provider. The results will be used to develop a content outline of a potential certification examination.

Your individual responses will be kept confidential and will be combined with those of other respondents. Only staff persons from the HFSA and PSI Services (assisting HFSA) will have access to the data collected. The combined data from this survey will be organized into a report that summarizes the results of the survey and how the results were used to develop the certification exam. The survey will likely require an average of 15-20 minutes to complete. You can complete each part of the survey in separate sittings as long as you use the same computer (or other device) each time.
