You are being invited to contribute to this survey because you have been a champion of food insecurity screening in clinical settings, using the Hunger Vital SignTM. The Food Research and Action Center (FRAC) and Children's HealthWatch are now seeking to better understand your efforts and gather collective lessons for screening to address food insecurity.

Please note:
⁃ This survey will be used for research purposes. Participation is voluntary. You may stop participating at any time without consequence.
⁃ The purpose of the research is to better understand and promote best practices to help clinical and community health practitioners ‘screen and intervene’ effectively. These data will help inform a white paper that will be featured in an American Academy of Pediatric's (AAP) toolkit currently under development by FRAC.
⁃ Participation will be limited to this survey.
⁃ You have the choice to remain anonymous or disclose personal identifying information if you would like to maintain the possibility of your organization’s work being highlighted. If you disclose personal identifying information, a possible risk of participating is the unintentional loss of confidentiality. We do not foresee any additional risks.
⁃ You will not receive any payment or incentive for participation.
⁃ Participating will have the benefit of improving our collective understanding of best practices for using the Hunger Vital SignTM and intervening effectively to address food insecurity.
