Introduction

The purpose of this survey is to identify leadership and team behaviors of others that help or hinder trust and engagement so that we can create action plans to strengthen working relationships. The results will be anonymous; however, to identify if there are any organizational factors we are also asking you to identify if you are a part of the District executive team or not, how long the DHD leader has been in place in that role and how long you have been in your role. If you feel there are other organizational factors affecting your involvement in decisions and engagement, you can list them in the open-ended comments section. 

There are five (5) short parts to this survey. The first part includes organizational factors related to your team and how long you and your manager have been in your positions. The second part assesses 16 leadership behaviors you would like to see more of from top management (i.e., DHD, supervisor). The third part assesses 12 team leadership behaviors you would like to see more of from top management (i.e., DHD, supervisor). The fourth part contains an open-ended question asking you to list one to two leadership or team behaviors you would like to see more of from top management. The fifth part is also an open-ended question asking you to list actions you might try to influence a better engagement outcome. All answers are anonymous.
