HAP and PA-HEN Patient-and-Family-Centered Care Assessment
This assessment is designed to assist The Hospital & Healthsystem Association of Pennsylvania (HAP) and the Pennsylvania Hospital Engagement Network (PA-HEN) understand the current practices around patient-and-family-centered care that are present in Pennsylvania hospitals and health systems. This assessment will also identify opportunities for education, technical assistance, and networking to support hospitals and health systems to develop a system of health care which supports personalization of care, as well as full engagement of patients and families.
