Professional opinion inquiry - dissertation study on US nuclear fuel reprocessing policy

* 1. Consent Form

My name is Yenting Tiffany Wu, and I am a doctoral student at National University (NU).

I’m asking you to take part in a research study about reprocessing nuclear fuel for critical infrastructure security. The name of this research study is “Cost-Benefit Analysis of Nuclear Fuel Reprocessing in Critical Infrastructure Security.”

You may participate in this research if you meet all of the following criteria:

1. You are age 18 or older.

2. You have worked in the American nuclear industry for 2+ years.

3. You have at least a basic technical understanding of nuclear fuel cycles.

I hope to include 20 people in this research.

Please read this form carefully and ask any questions you may have before agreeing to take part in this study.

What you will be asked to do: If you agree to participate in this study, you will be asked to do the following activities:

1. Participate as an individual not representing any organization.

2. Answering an asynchronous survey questionnaire consisting of three open-ended questions from a professional point of view.

During these activities, you will be asked questions about:

1. Your views on the energy security in the United States.

2. Your views on the French nuclear cycle.

3. Your views on the American nuclear cycle.

Risks: There are no foreseeable risks or discomfort associated with this study. You can still skip any questions you do not wish to answer, skip any activity, or stop participation at any time.

Benefits: If you participate, there are no direct benefits to you. This research may increase the body of knowledge in the subject area of this study.

Recording: There will be no audio or video recording for this study.

Confidentiality: I will keep the survey answers of this study private and take reasonable measures to protect the security of all your personal information. In any report I make public, I will not include the information that will make it possible to identify you. I will securely store your data for 3 years. Then, I will delete electronic data and destroy paper data.

Taking part is voluntary: Participation in this study is completely voluntary. You may quit at any time.
