2017 Benefit Survey

Goal of this Survey:

The City of Detroit strives to provide valuable, comprehensive and affordable benefit programs.  We are asking each employee to complete a benefit survey to let us know how satisfied you are with the current benefits offered, the cost of those benefits, and determine what is important to you.  We will use information gathered from this survey to assist in making decisions on additional benefits we offer in the future.  Please note that we will continue to offer the plans which were negotiated as part of the Plan of Adjustment without changes.  Each year we review our current benefit programs, particularly health and dental plans to ensure they live up to the goals of meeting our employee needs. If you have any questions, please contact the City of Detroit Human Resources-Benefits Administration Department and one of the representatives can help you.  Please complete the survey by March 22, 2017.
 
Definitions:

In this survey we use certain terms and abbreviations that we will clarify here.

Cost sharing: This is the amount that you pay to your provider when you have a service.  It includes deductibles, coinsurance and copays.

Deductible: This is a fixed amount that you owe for covered health care services before your BCBSM coverage begins to pay.  Services like inpatient hospital stays, surgery and diagnostic tests are subject to the deductible.

Coinsurance: This is a percentage amount that you owe for covered health care services typically after your deductible has been met.  Services like inpatient hospital stays, surgery and diagnostic tests are subject to coinsurance after the deductible has been met.

Copays: This is a fixed amount that you owe for covered health care services.  Services like office visits, emergency room visits and prescription drug scripts are subject to copays.

Premium: The amount you pay for your health insurance every payroll check.

High Deductible Health Plan: A health insurance plan with lower premiums and higher deductibles.  Usually the monthly premium is lower, but you have to pay more health care costs yourself (your deductible) before the insurance company starts to pay its share. A high deductible plan is combined with a health savings account. This allows you to pay for certain medical expenses with untaxed dollars.

Health Savings Account: A health savings account (HSA) is a tax-advantaged medical savings account available to taxpayers in the United States who are enrolled in a high-deductible health plan (HDHP). The funds contributed to an account are not subject to federal income tax at the time of deposit. These amounts roll over from year to year and remain available until spent.  These funds are also available to you for health care cost after you retire or leave employment with the City of Detroit

Coverage Tiers:  This describes the number and type of dependent that is enrolled with you on your coverage and determines the amount that you contribute for health care coverages.

   Employee Only (EE):  You only.
 
   Employee+One Dependent (Spouse or Child) (ED): You and one child or spouse only.
 
   Employee+More than one Dependent (Spouse and/or Children) (FM):  You and more than one child or You, your spouse and
   your child or children.
