HIV Planning Council:  Star Award Nomination Form

The HIV Planning Council wants to recognize people that go above and beyond for those affected by HIV. For the last seven years we have presented the Star Achiever Award to a volunteer that has stood out and taken the time to give back to the community! Five years ago, we added a Professional Star Award so that we can recognize the doctors, nurses, case managers, clinical researchers or any of the many professionals that have dedicated their life to the cause.
 
Please fill out the Survey Monkey and include a description of what makes this individual stand out to you! We are grateful to the people that give of themselves to improve the lives of people living with or affected by HIV!

Please complete this survey to submit your nominations by TBD. 

The Planning Council will review the nominations and present the award at a future Business meeting. Nominees cannot be current Planning Council members. Nominees will be voted on publicly via social media, email, and survey monkey. This will require nominees be shared on social media daily, and we will be contacting nominees to get their consent to this. 

The Planning Council recognizes that there are many HIV champions in Central Texas who work tirelessly to support those living with HIV in our community. We know that Austin is a diverse community and we want our nominees to reflect that, so we encourage everyone to be nominated, with special focus on minority populations such as people of color and trans people.
