  • Talk directly with your care provider or a team member if possible;
  • Submit electronically (this avenue);
  • Complete and submit a hand written 'Compliments & Complaints Form'. The form can be found on our website (www.windsorfht.ca) and in our waiting room.
Our commitment to you is to respond within (10) business days of receiving your completed Compliment and Complaint Form (either hard copy or electronically).
