Please be aware of several features of this training before filling out the application. This training is a 4 day training over 5 days, going from 6:00pm on day one to 1:00pm on day five (it either runs from Sunday - Thursday or from Monday - Friday). All trainers are required to attend the entire training.

Also, the training fee is $2,500 plus expenses which includes a $1,000 deposit that is required in order to hold your space in the training. Submitting this application is an acknowledgment that you understand these requirements.
