Produce Prescription Statewide Survey |
1. Introduction
The Ohio Department of Health (ODH) is conducting a landscape assessment of produce prescription (PRx) programs across Ohio. This survey aims to gather information such as what your program does, who your program aims to reach, your partnerships, and any challenges encountered in program administration. ODH plans to use the results to highlight existing programming strengths and gaps and to guide ODH actions to provide future support and funding strategies for these programs. At the end of the survey, there is an optional opportunity for you to upload and share any publicly available program reports if you are interested in doing so.
Produce prescriptions are a way for healthcare workers to prescribe fruits and vegetables, usually with free fruits and vegetables or vouchers to buy them, for patients with chronic diseases and lack of access to nutritious foods. The prescriptions may be used in the healthcare setting or at food pantries that distribute fresh produce, local farmers markets, brick-and-mortar or online grocery stores, or mobile markets. Many programs also offer nutrition education opportunities.
If you have any questions about this survey, please email the ODH Food Access Coordinator at casey.slive@odh.ohio.gov.
