How old is your dog, and what is his/her birthday? Is your dog spayed/neutered? (please note we require all male dogs to be neutered prior to joining playgroup, and all female dogs to be spayed prior to their first heat, OR to not attend playgroup while in heat for safety reasons) Is your dog friendly with other dogs? (feel free to be as specific as you like) Is your dog friendly with people? (feel free to be as specific as you like) Does your dog walk with a harness? (please note, we require all dogs to walk with a harness while under our care for safety reasons. If you do not have one, we can make some suggestions as to the ones we like, just let us know!). Do you have ID tags for your dog with your name and contact info? (Please note, we require that all dogs under our care have ID tags either attached to a collar (preferred) or to their harness). Does your dog have any health concerns (allergies, illnesses, etc.)? How do you feel about our dog spending part of their play group time off leash in an enclosed dog run? Where will we find your dog when we arrive? (crated, loose in house, etc.) Is your dog up to date on vaccines? Who is your vet? Is there anything else you would like us to know about your dog (s)? Question Title * 4. Tell us about your schedule: What time do you generally leave for work in the morning and return in the evening? What type of schedule are you looking for with Hoochie Poochie Club? How many days per week? Morning or afternoon play group, etc. (Please note, we require a set weekly schedule with a minimum of one day per week.) Question Title * 5. Tell us about your building: What are your cross streets? Do you live in a building with a doorman? If so, will we be able to get a copy of the key on a daily basis from them? If not, we require 2 copies of your key at our initial meeting (one is for your team member, the other we keep at our midtown office in the unfortunate event of a lockout). Is this doable for you? Does your building allow outside dogs to enter when we come to pick up your dog? This is very important to know, please speak with your doorman (if applicable) before submitting your application. Since we NEVER leave dogs tied up unattended, it is vital to have this information prior to adding your dog to our schedule. Every building is different. Some are very dog-friendly, but require dog walkers to use a service entrance, some allow only one or two outside dogs, etc. Please be as specific as possible. PLEASE NOTE: JUST BECAUSE YOUR BUILDING IS "DOG FRIENDLY" DOES NOT MEAN OTHER DOGS ARE ALLOWED TO ENTER. PLEASE CHECK WITH YOUR DOORMAN, MANAGEMENT, SUPER, ETC. TO GET THE OFFICIAL RULE/POLICY.
