Purpose of the Self-Identification Form (Page 1 of 3)
Goal of Survey:
To better understand the demographics of the Connected Minds membership and create a more equitable and inclusive funding program, we are inviting all members to provide voluntary self-identification data.
The survey contains 14 questions and will take approximately 5 minutes to complete.
Purpose of the Survey:
To streamline future self-identification requests, the self-identification information collected in this survey will be used for all future Connected Minds self-identifying purposes, unless otherwise requested.
For example, information collected from the self-identification survey will be used for 1) Program demographic planning and reporting, and 2) Developing EDI target priorities for Connected Minds opportunities.
Even if you have already self-identified or chosen not to in the past, we invite you to complete the self-identification form. Completing the survey today, will make submitting future funding applications quicker with one less step!
Choosing Not to Self-Identify:
For those choosing not to self-disclose there is an "I prefer not to answer" option available at the end of Page 1 (this will immediately end the survey) or after each question on Page 2 (providing you with complete autonomy in what is disclosed).
Reporting of Survey Results:
Self-identification information will be used to create a templated report that will present disaggregated proportions of the four federally designated groups (women, Indigenous people of Canada, Persons with disabilities, members of racialized groups), 2SLGBTQIA+, and French linguistic minority status as percentages. In the situation where less than 10 people self-identify, specific percentages will not be provided. These practices have been enacted to maintain the anonymity and confidentiality of respondents while also building a more diverse, equitable, and inclusive environment.
Accessing Self-Identification Information:
Self-identification information can only be accessed by the Connected Minds Program Director, Operations Manager, and EDI and Community Outreach Specialist, hereinafter referred to as CM staff.
Aggregate information will be shared for program planning and reporting, only. Personal self-identification information will only be accessed by CM staff for the final recommendation of applicants for the AASP program.
The AASP program will be applied to all current and future Connected Minds funding opportunities, after the adjudication committee has evaluated the merit of the applications for excellence. Applications ranked “Exceptional” will automatically be recommended for funding, applications deemed fundable will participate in the AASP process.
Connected Minds representational goals are based on York University’s institutional goals. Priority order is based on and York University’s institutional priority policies, and the applicant pool demographics.
If you do not wish to participate in the AASP program, you must select to opt out of participating during each funding call.
Information Storage on Server:
Self-identification information will be password protected and kept on a secure server hosted at York University accessible only to the CM staff. Information will remain in the server until 2037 as mandated by Canada First Research Excellence Fund (CFREF).
