Description of Learning Collaborative:
Where people live can directly influence their health. Neighborhood context, built environment, and reliable transportation play a crucial role in shaping health and well-being. Special and vulnerable populations often face additional barriers to care that are compounded by SDOH. Health centers are often well-equipped to address these SDOH because of their knowledge of the social, cultural, and economic needs of their patients.


Association of Asian Pacific Community Health Organizations (AAPCHO) will facilitate a learning collaborative for health centers serving special and vulnerable populations to explore effective strategies to address SDOH including transportation, environmental health, and neighborhood safety and sense of community. Participants will also have the opportunity to review and evaluate a closed-loop SDOH referral protocol that can be implemented at their health centers.


The learning collaborative consists of four 90-minute sessions, held on:

Session 2: April 24, 2024
Session 3: May 8, 2024
Session 4: May 22, 2024

Time: 9:30 - 11:00am HT | 12:30 - 2:00pm PT | 3:30 - 5:00pm ET

Learning Objectives:
  1. Understand the neighborhood and environmental SDOH that affect Asian Americans (AA), Native Hawaiians (NH), and Pacific Islanders (PI);
  2. Understand the unique considerations of screening for SDOH among AAs and NH/PIs;
  3. Learn CHC peers’ experiences screening for and addressing SDOH and identify best practices; and
  4. Develop tools and strategies to implement closed-loop protocols at their organizations.

Intended Participants:

Selection Criteria: Health centers and health center look-alikes will be prioritized for selection; Primary Care Associations (PCAs) and Health Care Controlled Networkers (HCCNs) are encouraged to apply with a member health center

Fee: No cost/free

Commitment: Accepted LC participants are asked to attend a total of 4 learning sessions in April and May 2024 and completed PDSA project
