SMCT Community Care & Wellbeing Program Survey

Thank you for taking the time to complete this survey, which has been created by Southern Metropolitan Cemeteries Trust (SMCT).

The purpose of this survey is to gather community feedback to help inform changes to SMCT’s Community Care & Wellbeing Program. Your input will enable us to shape the future program so that it continues to meet evolving community needs around grieving support.

The survey will take approximately 10-15 minutes to complete. The questions are based on your current experience of the Care and Wellbeing Program as well as suggestions for improvements. All responses and contact information will be kept strictly confidential.
