Award Application

The Healthiest State Awards recognize K-12 schools for supporting the physical, social, and mental well-being of students, staff, and families.

To qualify, schools must complete the 5-2-1-0 Healthy Choices Count! Assessment, which identifies strengths and areas for improvement in promoting healthy choices. Schools must then highlight how they implement three of the five assessment questions to create a healthy environment.

This award is for individual school buildings, not entire districts. District-wide applications will be disqualified.
