Vincent C. Gray Health Equity Award Nomination Form |
Award Criteria
The DC Hospital Association believes that to achieve health equity we must understand the challenges, aspirations, barriers, and realities of the community. To share the example of innovative approaches to equity and shine a light on leaders across the District of Columbia, DCHA created the Vincent C. Gray Health Equity Award. DCHA recognizes the importance of health equity and is working with its member hospitals and the community to ensure that disparities in health care quality and access are eliminated.
The Vincent C. Gray Health Equity Award is to honor and recognize individuals and organizations that are making health equity a reality in their community and leveraging engagement to improve health outcomes for those most impacted by health disparities. The award will go to an individual and hospital/organization that demonstrates contributions and efforts that reduce disparities in health care quality.
- The nominations must show demonstrated success in changing systems that impact health outcomes in one or more of the following areas: access to quality care, health disparities, equity, diversity, inclusion.
- Efforts must be targeted to improve outcomes for vulnerable groups most affected by health disparities (racial and ethnic minorities, individuals with disabilities, sexual and gender minorities).
- Must demonstrate a successful implementation of a systems change approach to reduce health disparities within the past five years.
- Illustrate measured improvement in health equity or reduction or elimination of disparities in health that adversely affect vulnerable populations.

DCHA will assess each application through a competitive process to identify the individual/organization that has had the greatest impact on health equity.
