We are conducting a survey to gather relevant data from the community so that we can be more objective in our meetings and decisions.

The survey is anonymous. Personally identifiable information is not shared, saved, or seen by anyone (email, name, and IP address are not collected).

The following are the 15 questions. Please help us serve you better by responding to all questions.
