High Impact Actions for Primary Care in Ontario
Primary Care Virtual Community Follow Up Survey

Thank you for your contributions in informing our emerging list of High Impact Actions for primary care within Ontario.

Our next step is to validate our Made In Ontario High Impact actions via our post session survey.

Our aim is to also identify the actions that are in our control as well as those that will require support at the point of care and system level.

Further, as we know many of the High Impact Actions are already being undertaken by individuals, practices and organizations, we want to learn from these examples across the province.

1.What actions most resonate to unleash the potential for primary care in Ontario?
2.Of the actions listed, what is within your control to lead or change?
3.Of the actions listed, what is not within your control to lead or change?
4.To advance Ontario's High Impact Action areas, what supports do you need from the Association of Family Health Teams of Ontario (AFHTO)?
5.To advance Ontario's High Impact Action areas, what supports do you need from the Ontario College of Family Physicians (OCFP)?
6.To advance Ontario's High Impact Action areas, what supports do you need from the OMA Section on General and Family Practice (SGFP)?
7.To advance Ontario's High Impact Action areas, what supports do you need from the Nurse Practitioners Association of Ontario (NPAO)?
8.To advance Ontario's High Impact Action areas, what supports do you require from policy or government?
9.Do you have any examples of organizations, individuals, or practices that are demonstrating the High Impact Action areas in Ontario? If so, please list them here.
